Forest Cemetery. When you enter the crypt after placing the Simmons Family Crest Piece A into the door, you will find a chest in the upstairs area. The key is inside the chest.

This key is used in Ada's Chapter 2, on the metal gate in the main crypt area below where you find the key. It gains you access to the Gallows puzzle area of the crypt.
