This creature is the result of a human being directly injected with the G-Virus. The human in question is none other than William Birkin, the creator of the G-Virus, who only subjected himself to his own creation in an effort to take back what was stolen from him. The G-Virus causes uncontrollable mutation, first targeting the arm that was heavily injured, causing it to swell and mutate into something freakishly strong.
